Mosaic Medical is a non-profit organization operating Federally-Qualified Health Centers in Prineville, Bend, and Madras, Oregon.
The mission of Mosaic Medical is to improve the lives and health of individuals and families in the communities we serve.
As Federally-Qualified Health Centers, Mosaic Medical Prineville, Bend, and Madras receive an annual government grant that allows us to offer discounted services to qualified under and uninsured patients. We accept most commercial insurance, Medicare, and Medicaid.
Our patient benefits include:
· Primary Care Services (acute and preventive care, pediatrics, women’s health including prenatal care in Prineville and Madras, chronic disease management)
· Outreach and Interpretive Services
· Medication Assistance Program
· Coordination of Dental and Mental Health Services
